1. Have you previously been a member of a Pentecostal Assemblies of Canada church?

2. Have you accepted Jesus Christ as your personal Lord and Saviour? (John 3:3)

4. Are you filled with the Holy Spirit?

5. Have you read the constiution of New Life Church?

If so, are you in agreement with the constitution?

6. Have you read WHAT WE BELIEVE in the Membership Application Booklet? It is also known as the Statement of Fundamental and Essential Truths of the Pentecostal Assemblies of Canada.

If so, are you in agreement with this Statement of Faith?

If not, would you like to duscuss any areas of concern?

7. Will you support the Church with your faithful prayers? (Ephesians 6:18)

8. Will you support the Church with your regular attendance? (Hebrews 10:25)

9. Will you support the Church with tithes and offerings? (Malachi 3:10, 1 Corinthians 16:2)

10. Would you be willing to be involved in at least one ministry of the Church?

11. A member's Christian testimony should comply with the following standards:

- A clear and credible profession of faith in the Lord Jesus Christ as Saviour (Romans 10:9-10)

- Abstinence from "acts of the sinful nature...sexual immorality, impurity and debauchery, idolatry and witchcraft; hatred, discord, jealousy, fits of rage, selfish ambition, dissension's, factions and envy; drunkenness, orgies, and the like." (Galatians 5: 19-21); and profane language (James 3:9). Sexual immorality shall be interpreted to mean common-law martial relationships, pre-martial and extra-martial relationships (1 Corinthians 6:15-18, 7:1-2; 1 Thessalonians 4:3-8; Hebrews 13:4), and all forms of homosexual and lesbian activity.

Do you affirm that your Christian testimony complies with these standards?

12. It is further expected that a member's daily conduct and lifestyle should reflect Christ by:

- Manifestation of growth by giving of the fruit of the Spirit: "love, joy, peace, patience, kindness, goodness, faithfulness, gentleness, and self control." (Galatians 5:22-23)

- Avoidance of conduct that could tarnish a Christian's testimony including the habitual use of tobacco, non-medical drugs and alcohol, gambling and inappropriate or immoral entertainment through television, movies or videos, the Internet, etc..

By filling in my name below, I provide assurance that I will voluntarily withdraw my membership without causing any discord among members or adherents, if at any time:

- I fall out of regular attendance to services, without offering an acceptable reason; or

- I feel unable to work in harmony with New Life Church because of personal convictions

I fall into violation of the testimony, conduct or lifestyle standards set forth above.
